This is working example. Enter name, any additional information as a "course info" ( ESL 313, Grammar-Unit-7, etc. ) and any comments (optional). Type your email address and get copy of the test result with detailed report by email. This is a demo page. Real page doesn't send copy of the report to the student. The report is sent to the instructor and he/she can forward it to the student with his/her remarks, comments or suggestions.
